Douglas Woodhouse and Carlijn Meerkerk presented the improvements in care that participating hospitals in the Netherlands have achieved using the Plexus Frail and Elderly Program. This session was a joint session with ActiZ that took place during the 2010
International Forum on Quality and Safety in Healthcare in Nice, France.
The issues that were addressed by Plexus included:

How can elderly patients at risk of complications be identified in hospitals?

How can complications such as delirium, falls, malnutrition and decubitus ulcers be reduced or prevented
- What are the results that hospitals that have participated in the Plexus Frail and Elderly Program achieved?
Attendees included approximately 40 delegates from around the world with an interest in improving elderly care. During interactive discussions delegates raised issues, provided examples of programs that they have implemented and discussed the results that they have achieved. We were pleased with the enthusiasm that was clearly evident and the exchange of ideas and solutions, and thank all participants for their contribution. We also thank our co-presenters at
